If you qualify and wish to submit an application,
you may do so by completing and submitting the following form.
If you do not have a Sponsor or Seconder, please forward
the form and you will be contacted with a list of member names, some of whom you may recognize.
MEMBERSHIP FEES
THERE ARE NO ANNUAL MEMBERSHIP FEES. UPON ACCEPTANCE
OF AN APPLICATION BY THE EXECUTIVE COMMITTEE, A LIFE TIME FEE OF TWENTY FIVE ($25) IS REQUIRED WITH THE APPLICATION. YOU
WILL BE ADVISED BY PHONE OR EMAIL RE INSTALLATION, WHICH WILL BE AT THE AT THE NEXT REGULAR MEETING.
